What kind of a day is the Joker having when he has to resort to this?
Or maybe he was just doing a terror campaign just with Johnny, his whole life. Stealing his report card, itching powder in the diaper, tabasco on the pacifier, setting him up with sex partners with STDs in high school, making him eat Aramark at college... you never know.

Yeah, I'm thinking this was the first of many incidences for Johnny. The Joker is a master of mind games, afterall.
